| 内容記述 | 目的：X－4 年に Posterior Cortical Atrophy（PCA）を発症し，X 日に転倒により左大腿骨頸部骨折を受傷した症例に対し，立ち上がりと歩行の再建を試みた． 方法：本症例は X＋41 日時点で大腿骨頸部骨折による侵襲や廃用性筋萎縮の影響に加えて，PCA に伴う視空間認知障害などを中心とした高次脳機能障害のため指示・状況理解が困難であり，立ち上がりや歩行が全介助であった．そこで言語聴覚士と高次脳機能障害の評価を行い，本人の理解の得られやすい聴覚からの情報入力を活用した指示方法にて理学療法を行った． 結果：X＋51 日から聴覚刺激を中心とした指示方法を実施したところ，X＋61 日時点で立ち上がりが見守りで可能となり，独歩での歩行練習も可能となった．X＋84 日時点で歩行が独歩見守りにて連続 200 m 可能となった． 結論：PCA は緩徐進行性の神経変性疾患であり，視空間認知障害を中心とした高次脳機能障害を呈する．PCAであっても病態を理解し，残存する聴覚からの情報入力を活用した指示やリズム刺激を工夫することで，立ち上がりや歩行の再建が可能であった． |

| 内容記述 | Purpose: We present the case of a patient who fractured their left femoral neck after a fall on day X. We attempted to restore their ability to stand and walk, which was complicated by a 4-year history of posterior cortical atrophy (PCA). Methods: PCA caused higher brain dysfunction, mainly visuospatial cognitive impairment, in the patient, which led to difficulty understanding instructions and situations. At 41 days post fracture, the patient had invasion and disuse muscle atrophy. The patientʼs higher brain dysfunction was assessed by a speech-language therapist, and we devised a physical therapy program that was delivered using auditory input, which was more easily understood by the patient. As a result of the adapted program, the patient was able to stand with a watchful gait on X + 61 days and was able to walk with a watchful gait for 200 m continuously on X + 84 days. Conclusions: PCA is a slowly progressive neurodegenerative disease that causes higher brain dysfunction, mainly visuospatial cognitive impairment. |
