| 内容記述 | わが国医療保障の日本の柱の一つである国民健康保険の事業は、市町村が運営しなければならないこととされている。そこで市町村は国民健康保険の費用を賄うため、その住民に対して保険料を賦課徴収しなければならない。しかし各市町村の状況は千差万別であり、地域の医療水準や経済の実態によって、医療費に著しい差があり、保険料負担能力にも大きな格差がある。国民皆保険体制が発足して以来、政府も地方公共団体も、市町村相互間と個々の住民相互間に公平で合理的な保険料負担のあり方を模索してきた。一部は財政調整交付金制度として不十分ながら実施されているが、問題は解決されておらず、関係者の間でいくつかの提案がなされながらも成案は得られないまま、今日に至っている。本稿ではこれらの提案が行われた背景と問題点を政策史的に概観する。 | |||||

| 内容記述 | National Health Insurance, one of the two entities of medical care insurance systems in Japan, is managed by the municipalities (cities, towns and villages). The municipalipies assess and collect insurance contribution from their residents to cover the cxpences of the National Health and Insurance. But differences in economic and medical situation among the municipalities have caused large discrepancy in the medical cost level and ability of insurance contribution. Since everybody in this country was covered by a public insurance system in 1961, both the government and the municipalities have groped the way to an equitable and reasonable burden sharing system of insurance contribution among the municipalities and community residents. The system is put into effect, though not enough, by a Grant for Financial Adjustment. Many propositins have been made, but much remains unsolved. In this paper, I discuss the propositions, their problems and backgrounds, in Japan's social security history. | |||||
