| 内容記述 | 目的：本研究では，初妊婦が無痛分娩を選択するプロセスを明らかにし，助産師が妊娠期から継続的に寄り添う支援についての示唆を得る． 方法：無痛分娩を選択する妊娠後期の初妊婦 8 名を対象に半構造化面接を行い，KJ 法を用いて分析を行った． 結果：初妊婦が無痛分娩を選択するプロセスに影響する要因は，【月経は不快な体験・母親の反応からも『おめでたい』ものとして受け入れがたい】【『おめでたい』はずの出産は痛い思いをしたくない】【自分を理解してくれる家族や友人の勧めで背中を押されたような】【情報に右往左往しながら医療者の説明に太鼓判を押された】【『出産の痛み』の体験を巡り自問自答し，葛藤する】【無痛分娩はやはり自分の希望を叶えてくれる唯一の方法】の 6 つが抽出された． 考察：助産師は，初経も含めた月経の情報も把握し，セルフケアの可否や薬剤や医療介入への抵抗の有無をアセスメントして寄り添う必要がある．そして女性が自信をもって最良の分娩方法と考える無痛分娩を選択できるように支援する必要がある． |

| 内容記述 | Purpose: To clarify primiparasʼ process of choosing painless delivery as a basis for providing continued midwifery support from the pregnancy period. Methods: Semi-structured interviews were conducted with 8 primiparas during the third trimester of their pregnancy, who had chosen painless delivery, and the obtained data were analyzed using the KJ method. Results: Six factors were identified to influence primiparasʼ process of choosing painless delivery: [perceiving menstruation as an unpleasant experience/finding it difficult to accept painful delivery as “a celebrated event” also in terms of maternal responses], [desiring to avoid pain during childbirth, which is supposed to be “a celebrated event”], [feeling encouraged by family members and friends who understand me], [being confused by information, but becoming convinced after receiving explanations from health professionals], [questioning and feeling conflicted about the experience of “labor pain”], [coming to the conclusion that painless delivery is the only method that fulfills my wishes]. Conclusion: It may be necessary for midwives to collect information regarding menstruation, including menarche, and assess the feasibility of self-care and degree of resistance to drugs and medical interventions when supporting pregnant females. Furthermore, such support should help these females confidently choose painless delivery, if they consider it as the optimal delivery method. |
