We adopted a team approach in a hospital ward to help a man in his nineties maintain sitting. The man had had a hemorrhagic cerebral infarction of the right middle cerebral artery region, and developed severe left hemiplegia and exhibited the Pusher sign. At the time of the initial evaluation, the patient required slight assistance to maintain sitting because of the Pusher sign. In addition, he exhibited poor alertness due to poor sleep at night and reduced overall strength from being confined to bed for a long time. These factors inhibited rehabilitation to maintain sitting and ameliorate the Pusher sign. Therefore, the physician, nurse, care assistant, and other rehabilitation staff took all the problems into consideration and sought for interventions to solve them. These included administration of a sleeping drug, extending the time spent sitting up, and toilet guidance, all conducted in the hospital ward. The rehabilitation approach was aimed at increasing the overall strength, practicing the movements for activities of daily living, and addressing the Pusher sign. As a result, the patient became able to maintain sitting for at least 20 minutes with observation and the amount of assistance was reduced. These results indicate that the team approach in the hospital ward was useful to improve the effectiveness of rehabilitation.","subitem_description_type":"Abstract"}]},"item_10001_description_6":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"　リハビリテーションを実践するにおいて多職種連携によるチームアプローチは重要である．今回，右中大脳動脈領域の出血性脳梗塞を発症し重度左片麻痺，Pusher 現象を呈した90 歳代男性の症例に対し，座位保持獲得のため病棟内チームアプローチを実践した．初期評価時，Pusher 現象によって座位保持に軽介助が必要な状態であった．また夜間不眠による覚醒不良や長期臥床による全身筋力の廃用が著明であり，座位保持獲得やPusher 現象に対するリハビリの阻害因子となっていた．そこで医師，看護師，介護士，リハスタッフ間で問題点を共有し，それに対しての介入を試みた．病棟では睡眠薬の服用や離床時間の延長，トイレ誘導などを行った．リハビリ介入では全身の筋力強化，ADL 動作練習やPusher 現象に対してのアプローチを行った．結果，座位保持能力が見守りにて20
