The rate of sequelae after stroke is high, andlong-term care is required. Cerebrovascular disease ranks first among causes that necessitate long-term care in the elderly.Using the government data and articles publicly available in Japan, the cost of illness (COI) for cerebrovascular disease wasestimated. Both direct (medical and non-medical) and indirect (mortality and morbidity) costs were included to assess theeconomic impact and characteristics of this disease. The total COI including direct and indirect costs for cerebrovasculardiseases was 6,587.7 billion yen. The long-term care cost, which was 2,470.8 billion yen, accounted for the largestproportion of costs. According to the Survey of Long-term Care Benefit Expenditures of 2013, the long-term care costs forcerebrovascular disease accounted for 26.9% of total nursing care costs which was 9.2 trillion yen (29.1% of total paymentwhich was 8.5 trillion yen).
