Participants: Twenty-three people, i.e., various professionals working in hospitals or long-term care agencies, clients and their families. Methods: Semi-structured interviews were conducted regarding current situations and issues in the involvement of clients in the collaborative process. The interview data were analyzed based on the grounded theory approach. Findings: Four factors, \"Mismatch in the purpose of collaboration,\" \"Poor identification of clients,\" \"Difficulty in sharing information\" and \"Level, behavior and attitude,\" were identified as factors inhibiting the involvement of clients in collaboration and decision-making processes, and information asymmetry was revealed between professionals and clients. Conclusion: Initiatives to overcome the asymmetry of information between professionals and clients are most important in encouraging clients to participate in the decision-making process.
