It can also be expected as a powerful tool and a platform, which improves care quality as well as efficiency, by enhancing collaborative healthcare. However, the PHR is still in its infancy stage in Japan. Since the term is used by multi stake holders in broad and different contexts, the concept of the PHR is not yet clearly defined or codified in a common guidance. This study will focus upon the current situation and the \"to-be model\" of PHRs in Japan with an internationally comparative light. Firstly, Japanese research papers relating to PHR were systematically reviewed by accessing and searching multiple academic databases. Secondly, in categorizing those papers, necessary conditions of PHR were examined and applied in order to make international comparisons. In addition to Japanese research papers, an interim report of \"Dokodemo Mai Byoin(DMB)\" released in May 2011 was also applied and reviewed. The DMB is the governmental five-year PHR project. As a result, only less than half papers mentioned \"system independency/ connectivity\" and \"patient centric concept/ design\". Also the DMB lacked the concept and functionality of \"keeping lifetime information\" in addition to the above two items.In conclusion, although the different environment of the healthcare system is to be concerned, the definition of the Japanese PHR should formed as a consensus in more patient centric way, in owning, accessing, and utilizing health information, including the governmental DMB project.","subitem_description_type":"Abstract"}]},"item_10001_description_6":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"パーソナル・ヘルス・レコード(PHR)は,患者が自らの医療健康情報にアクセス,管理,共有することを可能にする仕組みである.また,PHRは協働の医療を促進し,医療や看護(介護)の質と効率化を改善する強力なツールやプラットフォームとしての期待も高い.しかしPHRは我が国に於いて未だ端緒に着いたばかりであり,その用語は様々な利害関係者により,広くまた異なった文脈で用いられており,PHRは明確に定義付け,或いは共通の指針として成文化されるまでには至っていない.この研究は,我が国の現状とあるべきPHRモデルについて,国際比較の視点も踏まえ,その定義を検証する. まず,筆者らは複数の研究論文データベースからキーワード検索によりPHRに関する論文を抽出し,レビューを行った.次に,整理の方法として,PHR国際比較基準を用い,その基準に照らし合わせて再整理し検証した.加えて,政府の提唱する日本版PHRである「どこでもMY病院」5カ年構想についても同様の比較基準で検証した.結果として,まず論文レビューについては「システムの接続性,独立性」と「患者中心の思想設計」が要件として過半の論文において考慮されていなかった.次に「どこでもMY病院構想」の内容についても,上記の2項目に加えて「患者の生涯継続保持」の思想と要件が欠落していた.結論として,諸外国との医療領域における背景の違いは考慮しながら,どこでもMY病院構想はもとより,PHRの定義として患者中心の思想,設計に即した形で,患者が自らの医療健康情報を生涯保持,アクセス,或いは活用する為の合意形成が必要である.","subitem_description_type":"Other"}]},"item_10001_full_name_3":{"attribute_name":"著者別名","attribute_value_mlt":[{"nameIdentifiers":[{}],"names":[{"name":"Sugiyama,
