| 内容記述 | 目的：ICD-11では，悪性腫瘍の分類は部位より組織型が重視されている．1入院における悪性腫瘍の組織型別の医療費の差を明らかにすることで，ICD-11による大腸癌の組織型別の分類を使用した新たな医療費の診断群分類を提案する． 方法：3施設において2018年1月から2019年12月の2年間に入院治療した大腸癌の患者1,193件を対象とした．分析にはDPCデータと院内がん登録データを使用した．1入院にかかる診療報酬点数が大腸癌の組織型別により違いがあるかを重回帰分析で統計的に調べた．また，1入院にかかる診療報酬点数の中央値が，組織型別でどのようなクラスターに分類することができるかを確認するためにデンドログラムを作成した． 結果：目的変数を1入院にかかる診療報酬点数，説明変数を大腸癌の組織型とした重回帰分析の結果，4つの組織型において有意差が認められた．また，デンドログラムでは，組織型で大きく2つのグループに分かれた． 考察：大腸癌においては，ICD-11による悪性腫瘍の組織型による分類を利用した新たな診断群分類の作成が可能である． |

| 内容記述 | Purpose: In the International Statistical Classification of Diseases and Related Health Problems 11th Revision (ICD-11), histology dominates the classification of malignant tumors, rather than tumor locations. By identifying the differences in medical expenses by the histological type of malignant tumor per hospitalization, we propose a new Diagnosis Procedure Combination (DPC) for medical expenses according to the histological types of colorectal cancer based on the ICD-11. Methods: The study was conducted in a total of 1193 cases of colorectal cancer treated in an inpatient setting during the 2-year period from January 2018 to December 2019 at three institutions. DPC data and hospital cancer registry data were used for the analysis. Multiple regression analysis was performed to determine if the medical service fee points per hospitalization differed statistically significantly by the histological type of colorectal cancer. In addition, a dendrogram was created to determine what clusters the median medical service fee points per hospitalization could be classified into according to the histological type. Results: Multiple regression analysis performed using the medical service fee points per hospitalization as the response variable and the histological type of colorectal cancer as the explanatory variable revealed significant differences among the four histological types of colorectal cancer. The dendrogram showed that the histological types could be classified into two major groups. Conclusion: For colorectal cancer, it is possible to create a new DPC according to the histological type of the tumor based on the ICD-11-based classification. |
