| 内容記述 | 目的：本研究は，数年後に導入が開始される国際疾病分類ICD-11に合わせ，医療行為についても現行の診療報酬区分から国際分類であるICHIを用いたDPC樹形図の開発を行うことを目的とした． 方法：対象は，2018年4月1日～2020年3月31日に研究対象施設を退院した患者で，DPC14桁160800xx01xxxx股関節・大腿近位の骨折のうち，ICD-10がS72$大腿骨骨折，手術がK0461大腿骨骨折観血的手術またはK0811人工骨頭挿入術に該当する患者を抽出した． 結果：重回帰分析の結果，在院日数と入院経路について因果関係が明らかとなった．現行のDPC/PDPSに用いられているKコードの分岐をICHIに置き換え，新たにDPCコードを決定する分岐の情報として，入院経路をICHIでコーディングし新たな樹形図を開発した結果，在院日数の中央値は家庭からの入院が有意に長くなった． 結論：DPC樹形図に入院経路情報を組み込むことにより，職種間の情報共有が可能となり，在院日数の適性化をはかる資料として有益であることが示唆された． |

| 内容記述 | Purpose: The purpose of this study was to develop a tree diagram also for the diagnosis procedure combination (DPC) system for medical acts from the current classification by the national fee schedule for medical acts, using the International Classification of Health Interventions (ICHI); this is intended at meeting the criteria of the International Classification of Diseases (ICD-11), which is expected to be introduced in a few years. Methods: As study subjects, the following patients were extracted from among those who had fractures of the hip joint or proximal femur under DPC 14-digit codes (160800xx01xxxx) and were discharged from their respective study institutions between April 1, 2018, and March 31, 2020: patients classified as ICD-10 Diagnosis Code “S72 Fracture of femur” and underwent surgery classified as “K0461 Fracture surgery (femoral)” or “K0811 Artificial bone head insertion.” Results: The results of multiple regression analysis revealed a causal relationship between the length of hospital stay and the hospitalization route. The branch number of the K code used in the current DPC/per-diem payment system (PDPS) was replaced with the ICHI. In addition, as the information on the branch number required to determine a new DPC code, we developed a new tree diagram by coding the hospitalization route with the ICHI. The results revealed that the median length of hospital stay was significantly longer in patients hospitalized from home. Conclusions: Incorporation of information on the hospitalization route into the DPC tree diagram facilitates multidisciplinary information sharing. Therefore, the tree diagram is suggested as being a useful tool for determining the appropriate length of hospital stay. |
