| 内容記述 | 高齢者福祉領域においては、介護保険導入も伴って、サービスを適切に利用するためのケアマネジメントの重要性が言われている。病院におけるケアマネジメント、つまり退院計画も保健・医療・福祉サービスの連携の観点から、その必要性が注目されている。そのようななかで、平成9年度および平成10年度の2年間にわたり国際医療福祉大学と大田原赤十字病院の共同事業として、同病院において退院計画が試行的に導入された。本稿はその退院計画実践の実践報告である。本稿は、退院計画導入の意義および事業の概要、退院計画ツール(ハイリスクスクリーニング基準、退院計画アセスメント・ケアプラン票)の作成過程およびその説明、実践過程の報告であり、最後に見出された課題について報告する。 | |||||

| 内容記述 | Recently, care management is a topic of social work practice for the aged before care insurance system is started. And discharge planning is also a topic as the care management at the hospital. This paper is an investigational report on the trial practice of discharge planning. The investigation was made by the project team from International University of Health and Welfare and Otawara Red-Cross Hospital for 2 years ; 1997 and 1998. After digesting significance of discharge planning, we report about the outline of investigation, discharge planning system as a trial practice, and the discharge planning tool, which is constituted of the high-risk-screening criteria and assessment and care-plan sheet. And we advert to the task of the discharge planning system. | |||||
