Methods: We conducted semi-structured interviews with discharge-planning nurses at medical institutions in a prefecture and analyzed the interview’s content. The questions comprised three items: what types of changes and effects were felt upon commencing discharge support from before admission; for what types of patients and which medical departments are discharge support from before admission more effective; what is required before admission to make such discharge support more effective; and what are the issues involved? Results: In the interview of nine discharge-planning nurses, it was revealed that they found that the pre-admission interviews serve “to improve the patients’ self-management ability and understanding of the admission plan,” “to conduct early screening by and obtain cooperation among medical professionals,” and “to share and ascertain various patient information by medical professionals.” Pre-admission interviews also allowed discharge-planning nurses to provide support to patients for decision making. Furthermore, “the inadequacy of discharge support by ward nurses,” “dilemma of implementing discharge support on an outpatient basis,” and “the burden of a new support system on the patient” were perceived as issues. Conclusion: Discharge-planning nurses perceived pre-admission interviews as methods to prepare patients for hospital admission upon thoroughly understanding the medical care to be provided and to support patients’ decision-making.
